How to Send an Intake Form

So, you've created a custom intake form and you're ready to send it to your client. Or maybe you'd just like to send SelfDecode's premade Health Overview survey! This article will show you exactly how to do it.

1. Select the client on your My Clients page by clicking their name.

2. Select Questionnaires > Intake Forms.

3. Click the icon under Actions next to the intake form you want to send. 

4. Fill out their email if it's not already saved to their profile. Then, click Send. The form has been sent. 

You can also send the same intake form to several clients at once. Here's how to do it:

1. Select Professional Tools > Intake Forms from the top of the screen.

2. Click the icon under Actions next to the intake form you want to send. 

3. Select the clients you want to send it to. Click Send Intake Form. The form has been emailed to your clients!
